Support Worker Community Offer Plus

Location: St Albans Community Centre, St Albans Road, Smethwick

Pay: £12.50 per hour

Hours: 37hrs per week/ 35hrs per week/16hrs per week
main hours 9.00am till 18.00pm with occasional evenings till 20.00pm and weekend work on a rotation basis.

Duration: Fixed term until 31st December 2026.

Job Purpose:
Join our team as a Community Offer and Integrated Service Support Worker and make a real difference in people’s lives! In this rewarding role, you’ll provide compassionate support to the individuals in need, helping them navigate challenges, build independence and achieve their goals.
Whether it’s offering encouragement, support, advice and guidance or connecting individuals to vital resources, your work will have a profound impact.
The role will:
• Facilitate person centred support planning and co- ordination of input from different organisations, groups and professionals.
• Provide information/advice to individuals about a range of services to promote health and wellbeing and maintain independence.
• Facilitate access to services available in the community.
• Build relationships with primary care the local voluntary sector and community groups.
• Develop a knowledge of local services to enable the individual to access a range of services to meet their needs.
• Ensure individuals are engaged and connected with their local community and other organisations to make best use of resources.
• Support individuals to access community care assessments as well as carers’ assessments, where potentially eligible, monitoring outcomes of assessments and any follow up actions.
• Ensure Partnership working and a collaborative and collective approach is always taken.
• Where appropriate, actively participate in practice level multi- disciplinary team meetings.
• Identify when there is a need for urgent action or for a step-up in care and alert the relevant professional(s).
• Keep accurate and up-to-date records of contact with individual

Essential Preferred – access to car and driving license with business insurance

Qualifications Health, Social Care level 2 or Information and Advice qualifications

Experience Experience of working in health, social care or information and advice, in direct contact with service users in a paid capacity
Experience and a proven track record of working with people from diverse backgrounds
Experience of working with health and or social care professionals at a variety of levels either in health, statutory, voluntary or independent sector
Experience of working with people with multiple health issues and providing one to one support.

Skills – IT skills including database use
Compile letters and emails and have a good standard of English to GCSE level.

Knowledge
Understanding of issues affecting people and the ability to communicate in a sensitive way.
Ability to motivate and support individuals with complex care needs to engage in new activities.
Excellent communication skills, both verbal and written.
Ability to work in a person-centred way to identify and address the needs of people living with multiple long-term conditions and formulate support plans.
Ability to identify other issues affecting the persons health, such underclaiming of benefit, debt, poor housing, physical inactivity as well as other life-style issues and low mental health.
Commitment to continuous improvement to ensure the delivery of a high quality service.

Contract: Fixed term 24 months (31st December 2026)

Pay: £12.50 per hour

Expected hours: 16 – 37 per week

Schedule:
• Monday to Friday 9am – 8pm
• Weekend availability

Education:
• GCSE or equivalent (preferred)

Experience:
• Training: 1 year (required)
• Health and Social Care: 1 year (preferred)

License/Certification:
• Driving License (required)

Work Location: St Albans Community Centre, Midland Metropolitan Hospital, Murray Hall Community Trust.
